
This program convenes community members, caregivers, advocates, and stakeholders to engage in education, dialogue, and collaborative problem-solving around aging services, long-term care, and the protection of vulnerable adults. Sessions strengthen systems of carcommunity response.
Through South Carolina Aging Matters Consultancy, Ms. Jackson provides independent case management and advocacy, working collaboratively with professionals and entities to support the safest possible outcomes for vulnerable adults and veterans. Services include assisting with return-home planning or transition-from-home decisions, coordinating with law enforcement, hospitals, nursing homes, and community providers, and helping to develop and implement individualized safety plans. Drawing on her background as a former Regional Long-Term Care Ombudsman and Lead Adult Protective Services Investigator with the South Carolina Department of Social Services, Ms. Jackson supports informed decision-making that prioritizes dignity, safety, and overall well-being.
